This program reflects our commitment to partnering with patients who are comfortable sharing their story and transformation for educational and promotional purposes. Patients who are selected and awarded through this program may be invited to participate in documentation, depending on the needs of our educational and marketing initiatives.
Nothing will be published or shared publicly without your explicit, formal written consent. Before any public use or publication, you will be consulted and provided with the materials for your final review. A patient's willingness to share their story will not factor into their eligibility for the grant. Choosing not to participate in the documentation will not impact a patient's eligibility to receive an award if they meet all other criteria.If you are eligible, your Hybridge doctor will discuss this opportunity with you at your consultation.
